Losartan + Hydrochlorothiazide (Cozartan H) 50/ 12.5 mg tablet is a combination of two drugs that help keep your blood pressure under control. Losartan helps in reducing blood pressure by relaxing the blood vessels. Hydrochlorothiazide removes excess water and salts from the body through urination, thus reducing the amount of water in the body and lowering blood pressure.

How To Take
This medicine may be taken with or without food. Try to take it at the same time each day.
Losartan + Hydrochlorothiazide must be taken regularly for it to be effective. Continue taking this medicine even when you feel better. Do not stop taking it unless instructed by the doctor.
Take Losartan + Hydrochlorothiazide precisely as directed by your doctor or according to the instructions on the label. Do not take more or less than instructed by your doctor.
Dosage
Adult: Per tab contains Losartan 50 mg and Hydrochlorothiazide 12.5 mg: 1 tab once daily. Up to 2 tabs once daily 2-4 weeks later if needed. Maximum of 2 tab/day.
Missed Dose
If you miss a dose, skip the missed dose and return to your usual dosing schedule. If you often forget to take your medicine, let your doctor and pharmacist know.
Do not double a dose under any circumstances.
Overdose
Signs of overdose may include the following:
- Hypotension
- Tachycardia

Contraindications
People with the following medical conditions should not take Losartan + Hydrochlorothiazide:

Warnings
Do not use Losartan + Hydrochlorothiazide in people with the following conditions:
- Allergy to sulfonamide-derived medicines
- Unable to pass urine
Inform your doctor and pharmacist if you are taking any of these medicines:
- Aliskiren
- Digoxin

- Colestyramine
- Colestipol
- NSAIDs
- Lithium
- Potassium supplements

How Does It Work?
Hydrochlorothiazide increases renal sodium and chloride excretion and reduces the cardiac load. Losartan blocks the actions of angiotensin II of the renin-angiotensin-aldosterone system. This drug and its active metabolite selectively block the vasoconstrictor and aldosterone secreting effects of angiotensin II. The two drugs exert additive effects in hypertension.

- Existing electrolyte disturbances
- Hepatic cirrhosis
- Severe hepatic failure
- Edema
- Elderly (>75 yr)
- Renal impairment
- Hepatic impairment
- Diabetes
- Gout
- Hyperlipidemia
- Hyperuricaemia
- ECG: LVH and ventricular ectopics extrasystoles)
- Volume depleted patients
- Patients on diuretics and salt restriction
- Renal artery stenosis
- Aortic and mitral stenosis
- Monitor potassium concentration
- Discontinue before performing tests for parathyroid function.
- Avoid alcohol.
