How To Take

This medicine should be taken with food or immediately after a meal. Try to make it at the same time each day.

Take Ivabradine precisely as directed by your doctor or according to the instructions on the label. Do not take more than instructed by your doctor.

Dosage Ivabradine (Ivasmart 5) 5 mg

Adult:  Chronic stable angina pectoris in coronary artery disease patients w/ normal sinus rhythm: Initial: ≤5 mg twice a day. Increase if necessary, to 7.5 mg twice a day after 3-4 weeks. Titrate downward to as low as 2.5 mg twice a day, if resting heart rate is <50 beats/min or patient experiences symptoms of bradycardia. 

Chronic heart failure: Initial: 5 mg twice a day. After two weeks, adjust the dose to achieve a resting heart rate between 50-60 beats/min. Maximum of 7.5 mg twice a day.

Missed Dose

If you miss a dose, skip the missed dose and return to your usual dosing schedule. If you often forget to take your medicine, let your doctor and pharmacist know.

Do not double a dose under any circumstances.

Overdose of Ivabradine (Ivasmart 5) 5 mg

Signs of overdose may include the following:

  • Severe and prolonged bradycardia

Seek medical help immediately.

Contraindications

People with the following medical conditions should not take Ivabradine :

  • Resting heart rate <70 beats/min before treatment
  • Cardiogenic shock
  • Acute myocardial infarction
  • Severe hypotension (<90/50 mmHg)
  • Sick sinus syndrome
  • Sinoatrial block
  • Unstable or acute heart failure 
  • Pacemaker dependent 
  • Unstable angina
  • 3rd degree AV block
  • Severe hepatic impairment
  • Pregnancy and lactation
  • Concurrent use with potent CYP3A4 inhibitors (e.g., azole antifungals, macrolides, HIV protease inhibitors or nefazodone)
  • Moderate CYP3A4 inhibitors (e.g., verapamil or diltiazem)

Side Effects

Ivabradine may have the following side effects:

  • Luminous phenomena in the visual field (phosphenes) 
  • Blurred vision
  • Bradycardia
  • Other cardiac arrhythmias
  • Syncope
  • Hypotension
  • Asthenia
  • Fatigue
  • Headache 
  • Dizziness 
  • Nausea 
  • Constipation
  • Diarrhea

Warnings

Do not use  in people with the following conditions:

  • Heart disease
  • Severe liver disease
  • Very low blood pressure (blood pressure less than 90/50 mmHg)
  • Heart rate maintained exclusively by pacemaker

How Does Ivabradine (Ivasmart 5) 5 mg  Work?

Ivabradine is a heart rate lowering agent that works by selectively and specifically inhibiting the cardiac pacemaker, which is current responsible for controlling the spontaneous diastolic depolarization in the sinus node and regulates heart rate.

Uses 

Ivabradine is used  to treat the following conditions:

  • Chronic stable angina pectoris in coronary artery disease patients with normal sinus rhythm
  • Chronic heart failure

Special Precautions and Connected Warnings

Take special precautions if you have the following conditions:

  • Patient with retinitis pigmentosa 
  • Second degree AV block 
  • Congenital QT prolongation 
  • Atrial fibrillation or other cardiac arrhythmias that interfere w/ sinus node function
  • Severe renal impairment (CrCl <15 mL/min)
  • Avoid alcohol and grapefruit juice

Storage Conditions

  • Store at 25°C.
  • Store in a cool, dry place away from the reach of children.
  • Do not use ivabradine that is expired or out of date
