Vildagliptin + Metformin (Vildasmart M 50/850) 50 mg + 850 mg tablet is a combination of two drugs used to treat type 2 diabetes, where there is a gradual loss of capacity of the pancreas to produce enough insulin. The medicine keeps your blood sugar in control. It is meant to be taken with exercise, a healthy diet, and regular blood sugar monitoring.

How To Take
This medicine should be taken together with food or immediately after a meal. Try to take it at the same time each day.
Your doctor will decide the dose of this medicine. Your doctor will advise you on the course of the treatment, depending on your condition and response to the medication.
Metformin + Vildagliptin must be taken regularly for it to be effective. Continue taking this medicine even when you feel better. Do not stop taking it unless instructed by the doctor.
Take Metformin + Vildagliptin precisely as directed by your doctor or according to the instructions on the label. Do not take more or less than instructed by your doctor.
Dosage
Adult: Each tab contains vildagliptin (mg)/metformin (mg): 50/500, 50/850, or 50/1,000: 1 tab twice a day based on patient’s current regimen, effectiveness, and tolerability.
Missed Dose
If you miss a dose, skip the missed dose and return to your usual dosing schedule. If you often forget to take your medicine, let your doctor and pharmacist know.
Do not double a dose under any circumstances.
Overdose
If you think you may have used Vildagliptin + Metformin more than the usual dose, please seek medical help immediately.
Contraindications
People with the following medical conditions should not take Vildagliptin + Metformin:
- Acute metabolic acidosis (e.g., lactic acidosis, diabetic ketoacidosis)
- Conditions which may alter renal function (dehydration, severe infection, shock)
- Acute or chronic disease may cause tissue hypoxia (e.g., cardiac or resp failure, recent heart attack, shock)
- Undergoing surgery
- Acute alcohol intoxication
- Lactation
- Severe hepatic and renal (GFR <30 mL/min) impairment
- Intravascular administrationof iodinated contrast agents
Side Effects
Vildagliptin + Metformin may have the following side effects:
- Dehydration
- Acute pancreatitis
- Bullous and exfoliative skin lesions
- Decreased vitaminB12 levels
- Hypoglycemia
- Rarely, hepatic dysfunction (i.e., hepatitis)
- Dizziness
- Tremor
- Headache
- Fatigue
- Nausea
- Hyperhidrosis
Potentially Fatal:
- Rarely, lactic acidosis
Warnings
Do not use Vildagliptin + Metformin in people with the following conditions:
- Severe kidney disease
- Liver disease
- Excessive water loss from the body
- Severe infection
- Heart failure
- Recent heart attack
- Any type of metabolic acidosis
- Lactation
Inform your doctor and pharmacist if you are taking any of these medicines:
- Glyburide
- Enalapril
- Furosemide
- Cimetidine
- Ibuprofen
- Celecoxib
Inform your doctor and pharmacist if you are taking any other medicines, including herbal tonics such as traditional Chinese medicines, supplements, and non-prescription medicines.

How Does It Work?
Metformin is a biguanide antidiabetic agent that decreases hepatic glucose production, improves peripheral utilization and glucose uptake by increasing insulin sensitivity, and delays intestinal glucose absorption. Vildagliptin is an inhibitor of dipeptidyl peptidase-4 (DPP-4), thereby increasing the concentration of glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP), by this, it enhances glucose-dependent insulin secretion and reduces glucagon production.

Special Precautions and Connected Warnings
Take special precautions if you have the following conditions:
- Patient with history of acute pancreatitis
- Mild to moderate renal impairment
- Pregnancy
- It is not intended for the treatment of type 1 diabetes mellitus and diabetic ketoacidosis.
- Avoid alcohol.
- You need to maintain a healthy diet and weight to help keep your diabetes under control.
Storage Conditions
- Store below 30°C. Protect from moisture.
- Store in a cool, dry place away from children.
- Do not use Vildagliptin + Metformin that is expired or out of date.
